As a full post acute continuum of care "campus", with skilled and assisted living services, we are uniquely capable of meeting the changing needs of your patients and today's challenging healthcare environment.

We have nurses, admission specialists, and billing specialists standing by to assist you in any way possible. Our goal is to make your job easier. Whether it is handling the transfer paperwork onsite at the acute hospital, visiting with the patient and family onsite, or resolving insurance obstacles, we recognize it is our job to make referring to us easy.

Further, we will take calls for referrals at any time, day or night. Yes, we mean 24/7!

We recognize the financial risk hospital readmissions pose and are fully prepared to partner with you in reducing them. We also view it as our responsibility to assist you in achieving your length of stay goals.

How to refer patients

To refer patients, simply do the following:

  • For assisted living services at Leisure Vale, call our business office at 818-244-2323 and let us know your interest in referring a patient of yours. If your need is urgent, you may reach our administrator 24/7 at 818-800-7840.
  • For skilled nursing services at Leisure Glen, call our business office at 818-247-4476 or our admission coordinator 24/7 at 818-220-1535.
